Animal Care and Use Program Review Form
The CCAC Animal Care and Use Program Review Form (PRF) is designed to allow institutions to thoroughly assess their own animal care and use program. All questions in the PRF should be answered accurately and completely. The information contained in a completed PRF is reviewed by a CCAC assessment panel prior to an assessment visit and is also used to produce the subsequent assessment report. The questions in the PRF are based on CCAC policy statements and guidelines and, as such, refer to components that must be included in all institutional animal care and use programs. Reviewing and completing the questions provides guidance to animal care committees (ACCs) on the elements of a sound animal care and use program. As part of the regular review of their institutional animal care and use program (see the CCAC policy statement on: terms of reference for animal care committees), ACCs must conduct annual visits of the animal facilities; Section 6 of the PRF can assist ACCs in this task and has been tailored for different types of animal use.
All seven sections of the PRF (including at least one of the forms in Section 6, if you have an animal facility) must be completed. Double-sided paper copies of the completed PRF, along with all pre-assessment documentation, should be sent directly to the assessment director, and an electronic copy sent to the CCAC office, three weeks before the assessment visit.
